Submit only one of these upload submissions per artist. Based on this information you will be approved or denied to tattoo by the Department of Health. Once the correct information is submitted, you will be invoiced for your permitting fees. Below are the links to the forms you will need to fill out, sign, & scan for return. These forms, BBP, and IDs can be uploaded at the bottom of this page. All necessary permitting information should be submitted no later than September 26, 2023. IF YOU ARE ATTENDING WITH SHOPMATES OR FRIENDS, PLEASE INCLUDE THEM IN THESE TWO APPLICATIONS. THESE FEES ARE CHARGED PER SHOP, NOT PER ARTIST. THE MORE ARTISTS IN YOUR SHOP, THE MORE YOU WILL SAVE ON FEES. WE WILL PLACE YOUR BOOTHS TOGETHER BASED ON THESE FORMS.
